Don't do it on speaker phone, its probably using an inband detector to turn the one played out the speaker phone into a DTMF tone too there by duplicating them. Chances are the phone is actually sending two, are you dialing on speaker phone by chance? If so don't. See if the problem goes away.

Seems there should be a dtmf guard time or something to set to filter this. The problem has happened a couple of times while in voicemail I delete the current message and FS deletes the current and the next as well. I may have bounced the button but if I did it was too short of a silence in between for me to hear.
