1) Depends on what the external application is trying to accomplish.
Outbound socket is probably the most powerful and flexible way to
accomplish call control from an external application but it is a steep
learning curve.

2) Are you transferring the user to an IVR from within the ESL application
or in the dialplan XML?
3) You can execute all dialplan from within ESL the same way as you can
from the dialplan XML
4) Yes. The question is only how much time and effort it would take to get
there.

If you are trying to create a calling card that can be refilled mid call
then check out mod_nibblebill.
