is it a physical or virtual host? It's quite common that with public
virtual hosting, your CPU resource is not guaranteed, and FreeSWITCH
may lose RTP packets or send them too late because it didn't have
enough CPU cycles.

You can analyze the quality of RTP stream with tshark, and also
there's voice analysis software by Sevena
(http://sevana.biz/products/aqua/).
