Michael,

Pastebin the necessary debug info. Check http://wiki.freeswitch.org/wiki/Reporting_Bugs for specifics on how to do collect and report all the data to pastebin. It would be good to see the dialplan and the debug log for each call. 

When you've collected all the data please reply here with the pastebin URL.
