I modified the Internal profile in
/usr/local/freeswitch/conf/sip_profiles/internal.xml to include:

   <param name="NDLB-force-rport" value="true"/>
The phones connect on port 5060 - nothing specical to config in the
<mac>-phone.cfg file for the phone; just host, port, user/pass.
