That log looks perfect. Note line #108: EXECUTE sofia/internal/1210@sip.agshealth.com hash(insert/10.10.10.20 -call_forward/1210/1010) That's inserting the CF info into the database. This is working. Please pastebin the debug log of whatever isn't working.
