with some programming effort, you could create a Web service where the
users would manage their conferences, and also dial-out to invite new
parties in. It's quite straightforward to do via ESL, it only needs a
good developer and a good plan for long-term support.

I would love to contribute to such a project if it's an open-source
effort with solid sponsorship.

In a minimalist approach, you can issue an originate command from
fs_cli, and it can also be easily wrapped into a simple PHP script. It
would be ugly, but will do its job :-)

Alternatively, you can build a simple IVR, so that the one who invites
a new person, dials into the IVR, gives the destination number, then
talks to the invitee, and after hangup the call is automatically
bridged into the conference.

So, there are tons of different scenarios :)
