That is because you set that custom ringback tone in the part of the
dialplan that executes for local extension calls.

Determine which <condition> matches calls to the PSTN and set the ringback
tone inside that <condition> block. Different parts of the dialplan will be
matched and executed for different calls.

But in some cases the PSTN provider might not send the necessary signaling
back to FreeSWITCH to play that tone, so you might be disappointed. Nothing
FS can do about that, I'm afraid.
