Look at line 107 of your paste:

Resulted in Error Cause: 34 [NORMAL_CIRCUIT_CONGESTION]

For whatever reason mod_freetdm thinks that port is in use. Try "ftdm dump
<span> <chan>" to see if there's any clues as to the state of the channel.
Also, what country is this? Do you have your tones properly specified?
