Take a sip trace on your box with tcpdump or preferably, tshark:
tshark port 5080
You will then see if you receive it.
In the config you sent, I don't see anything about register expiration, or NAT keepalive, so I would really recommend you add a ping every 30 seconds to your gateway config.
If the issue is there, it's quite easy to see:
unregister the gateway
register it again
make an inbound call right away
