You probably do not need to do anything with your firewall to facilitate
outbound calls. Inbound calls would need 5060 open and/or routed to the
proper host behind the firewall, as long as some ports for RTP (I
*think* the range is somewhere around 8000 udp).
A VPN most likely will avoid this issue assuming that you are not
firewalling between the vpn endpoints.
