Remember to turn on caching as much as possible.  You may give a  look
into FusionPBX, its xm handler has memcache support and you can put a
big TTL if your dialplans dont change.  If you are  not going to do
any kind of fancy load-balancing you can place .db files into a
ramdisk.

But if you want to do billing later, you must use your CDR carefully,
an external DB is the best.
