After a brief discussion during the conference call early today
William King urged me to send a message to the list about the profile
parameter auth-all-packets.

I did some testing with auth-all-packets=true and the following
methods are not challenged OPTIONS, SUBSCRIBE, INFO and MESSAGE.
So auth-all-packets does not auth all packets.
The documentation has been updated to reflect the current behaviour of
auth-all-packets.
I doubt there is much interest in having OPTIONS requests challenged
but maybe SUBSCRIBE should be.
A parameter auth-messages already exists and similar ones could be
made for other methods.
