We need more information.

Make the call.  Then copy the freeswitch.log file (including all the lines
from the time the call was first received until it is sent to the ipbx
extension).  Use http://pastebin.freeswitch.org/.  Do not flood the mailing
list.  Then we can have a detailed look.
