The GXW4104 is an FXO gateway, you should not be registering it as an extension. You need to set up a gateway in freeswitch, and an inbound and outbound route. This may help you https://wiki.freeswitch.org/wiki/Traditional_Gateway_connecting or take a look at page 79 in the book.

Dear all,

I have a grandstream GXW4104 gateway (voip-landline gateway) and i have problems in installing it. 
The gateway (GW) registers as an extension (1005) to my server. I need to send landline calls to extension 1005 and change the SIP header (effective caller ID) to the new landline number (ex: 0352209xxx) 

could you help me with the dial plan?
