If you didn't already fix your issue then perhaps you could use this:
http://wiki.freeswitch.org/wiki/Mod_commands#uuid_broadcast

The uuid_broadcast API lets you execute an arbitrary dp app on a channel.
You could execute the att_xfer app on the channel in question. It *should*
work just as if it had been called from the dialplan.

Let us know how it goes.
