Looks like 1003 at pbx is hanging up - can you adjust your logging to 
include the SIP signalling between (I presume) the handset and 
FreeSWITCH (the "internal" SIP profile)? We seem to be seeing 
FreeSWITCH<>Provider but not Handset<>FreeSWITCH?
