could you get the latest GIT f69f4a77477d59ee2734dce74f5c984bd9a4d678 or better
and repeat that trace but do the following too

fsctl debug_level 1

if you can reproduce it with that try another one with the additional:
sofia loglevel all 9

and post it all to jira

It looks like some sort of race with bypass media mode and queued indications
