Actually, that's very interesting. The start_dtmf app looks for in-band
DTMFs, so if you have to have that in order to hear digits from the other
end then the other end most definitely isn't doing RFC2833.

For the sake of testing, try sending in-band and see how the other end
reacts. Might want to check this out:

My guess is that the equipment along the way is futzing with things.
FreeSWITCH does break easily but it does find bugs in other VoIP systems
that it talks to... :)
