Yeah, the debug log didn't really say why the bridge failed. I'd try a few things: pastebin your relevant dialplan bits as well as whatever code you use to put the agents into the queue as members. Also, turn on SIP debugging at the console:
sofia profile internal siptrace on

Look to see if there's anything interesting or unusual in the SIP dialog. You can pastebin that as well.
