I had a similiar case (no media heard), but in the IMS/MGW environment (PSTN-to-IP call). Adding a SIP header

P-Early-Media=sendonly

when sending 183 response solved the problem. Check RFC5009. Also, executing 'pre_answer' explicitly after setting the header would be nice.
