I have seen a lot of threading issues with mysql + odbc, make sure you're using unixodbc >= 2.3. 

If you're using an older version you can set Threading = 2 in your /etc/odbcinst.ini as a workaround, but this is *not* recommended for production/high volume, upgrade as soon as possible.
