I have definitely seen this work in the past and nothing has changed related to this.  Its unlikely to be the problem, and your tests have other differences is my best guess.

I created a SIPp scenario to reproduce my issue, my SIPp scenario would answer with 200 OK to fs and FS would never ACK it.
Once I removed the compact headers, with the exact same scenario, FS started ACK'ing the 200 OKs...
