Hello Michael,
 
My hosting provider was filtering outgoing UDP packets having source port = 5060. Running FS on the different port eliminated the problem.

How were you able to determine that FS was not emitting any SIP traffic? Side note: do you have more than one NIC on this system and is mod_sofia binding to the correct interface?
