I just solved the issue.<br>
My ITSP set the max_forward header to a low number (5) so FS
dropped the packet in the nested IVR case (more than 5 transfers).<br>
I have forced the max_forward header to a higher number (65).<br>

The useful error that should say why its doing this is truncated
