Hello,
What I meant by disappearing is: an inbound call might fail now & 5 minutes
later NAT is fine.
There's no firewall so, I think it might be the OPTIONS pings.
Anyway, I will change the router just to see any improvement.

I don't understand what you are saying? What is disappearing? No, you never
want to send a fake packet.. there is a message flow that includes ack for a
reason. You need to address whatever is causing lost packets or you can't hope
to have coherent conversations over sip.
