Figured it out. You need to add the parameters using...
conference_auto_outcall_prefix={group_confirm_key=1,group_confirm_file=conference/conf-join.wav,ignore_early_media=true,call_timeout=60}

Q2. If you auto outcall and it goes into voicemail is there anyway to kick the offending conference leg? This is especially a pain with mobiles as invariably the only way to stop the voicemail from just sitting there recording your conference is to hang up?
you could use group_confirm to require participants to confirm they are human by pressing a digit.
