I also have issues in Windows after 1000-1500 ports bridge with 200-300 connect calls and 150 cps.  I see a lot of  reinvent. I am using imbedded.

It's almost like the threading is getting backed up and the events don't fire.

I am running in a ram drive with no sql, no cdr and presence not on.

Funny thing I found it's not a cps issue but more of port/channels issue. 

Always failed due to ports but never crash and no errors.

Did you comment out manage-presence ?  Also we have no real idea how well FS performs in windows under extreme load.

Going with the idea it’s IO related I did a db_cache status. I just disabled limit in my code by removing it but limit should be HASH not a database however I stills how a database.
