*Carriers:*
People speak very highly of flowroute <http://www.flowroute.com/>, who are
focused on USA. Their T38 for fax sounds like it works great.

For higher-scale origination, AnveoDirect <http://www.anveodirect.com/> has
an API to buy voxbone DIDs anywhere in the world. (Voxbone has a stellar
reputation.)
If you just need a few 2-channels DIDs you can use
localphone<http://localphone.com> (this
is my link<http://www.localphone.com/?rb=JPolW-_w1_JdRLa7iLscabG5_e5dCBsI5iX-zvR7lhU>),
they resell voxbone for most routes.

For competitive international termination, I usually use
voxbeam<http://www.voxbeam.com/>'s
premium route and net2phone's <http://net2phonewholesale.com> as a backup.

Anveo, voxbeam, net2phone have a pre-payment of $50-$100. Other carriers
include siproutes, alcazar, and xconnect but they require manual setup
and/or possible large minimums.
