Probably easiest to move the FXO gateway to same network as Freeswitch, however you can deal with NAT the same way you would when connecting to an ITSP, Freeswitch will see your FXO gateway like an ITSP. Read https://wiki.freeswitch.org/wiki/Nat

Dynamic IP, you can use dyndns.

The gateway is located after a NAT server with a dynamic IP of the NAT router. Any idea?

 The GXW4104 is an FXO gateway, you
 should not be registering it as an extension. You need to  set up a gateway in freeswitch, and an inbound and outbound  route. This may help you https://wiki.freeswitch.org/wiki/Traditional_Gateway_connecting

 I have a grandstream GXW4104 gateway (voip-landline gateway)  and i have problems in installing it. 
 The gateway (GW) registers as an extension (1005) to my  server. I need to send landline calls to extension 1005 and  change the SIP header (effective caller ID) to the new  landline number (ex: 0352209xxx) 
 
 could you help me with the dial plan?
