Tim,

enable xml_cdr (both legs), then look at one of those, and you'll see all the possibles fields you can enable in CSV.
Most of them are quite self-explanatory.

About quality, that depends on RTCP.
I don't think RTCP is completely implemented in FreeSWITCH yet, but I remember some people were talking about it recently.
You should search in the archives.
