Default dialplan? If not, pastebin your dp. At some point you need to set
the caller id name/number channel variables. Authenticated calls will have
those populated if you define them in the user directory, otherwise you will
need to manually set them in the dialplan, or at the very least modify the
default dialplan to have something more meaningful than all zeroes.
