Is it 5 minutes to the tee? I would do a packet capture and see what is
happening with your signalling. Sounds too short to be a re-INVITE but
who knows what your provider is doing. Get yourself tcpdump and
wireshark then run the following from the FS box:
tcpdump -s 0 port 5060 and udp -w /tmp/capture.pcap
Not sure if I'm mistaken but the sip callid should be the UUID that FS
generates. You can filter in wireshark on that and see the transactions
between FS and your provider. Look for repeats of BYE and ACK.
