This was exactly pointed out by Anthony Minessale on this response as shown
ablove.
If you know the uuid's of the two channels then you can do

The problem with a newbie like me is it is very hard trying to understand
and/or learn FS without studying from some examples even though the wiki on
http://wiki.freeswitch.org/wiki/Mod_commands#uuid_bridge uuid_bridge has
explained it (unfortunately, no samples/examples ). Iwas hoping someone here
who has implemented this can show me his/her dialplan on how to achieve
this.
