It favors the connection that leads to the internet, it's a
best-guess scenario unless you set it yourself when you have many
interfaces.
do you have "manage-presence" set to true? in your sofia profile, its
necessary for mwi.
connect to FS with fs_cli and enter the command:
/events message_waiting
and watch them to see what you get and see if anything is out of place.
also are you using the domain in the voicemail call?
<action application="voicemail" data="default ${domain} ${extension}"/>
for instance. It's crucial that the domain called to voicemail matches the
directory and the profile.
The default config sets domain to the same as local_ip_v4
