Is this a typo "managed-shared-appeareance=true" or is there an extra e in
appearance in your config?

OK, I don't know what happened there, here's another one with the
NOTIFYs. I'm on trunk rev 16633 and I have
"managed-shared-appeareance=true" on the internal profile. I'm just
making calls between internal phones.
