Between phones, when the media is peer to peer will be fine. When the
media is anchored to a media server running on hardware you need to
ensure the media server is not having any latency issues. Hence
Anthony's 2 points.

Media services (any platform/engine) having any latency or jitter
issues (due to timing especially) will result in what you have
experienced.
