Stas,

I guess we didn't scare you away from FS when you were at ClueCon! :P

I'm not sure about your first question, but your second question seems to
indicate that you have an older dialplan. You can test it out by dialing
"9996" instead of "9196" or "9999" instead of "9664". If the 99xx extensions
work then you have an older dialplan, in which case you should save any
modifications that you've made and then delete conf/dialplan/default.xml.
Then run "make samples" in your fs src directory to get the most recent
default.xml dialplan file.
