Have you looked over various example configurations on the wiki? see: SIP
Provider Examples
I'm a bit confused by your questions... to make voip calls work, you
basically need to register/authenticate with proxies/gateways to send and
receive calls through them. Sounds like you need to find out from your
provider's documentation as to how they expect you to connect with them.
Why are you trying to connect to the host behind the proxy rather than the
proxy?
